Making your appointment
When you call to make an appointment for your eye exam:

Medical history
On your first visit to Salazar Eyecare, we will ask you questions
about your health and your family medical history. We will also want to
know about any eye problems you have noticed.
It may be helpful to make some notes before your visit! Try to jot down:
- symptoms or problems with your vision
- blurred vision
- light flashes
- poor night vision
- pain
- irritation
- dates and treatments for past eye injuries/surgeries
- general health
- allergies
- chronic health problems
- diseases
- surgeries
- family history of eye problems
- any questions you have about your vision and eye health

What to bring
- glasses and/or contacts you are now using
- list of prescription and/or over the counter drugs you are taking
- health insurance card
- photo ID (driver's license)

General tips
- ask questions about anything you don't understand during your eye
exam
- make sure you understand any instructions and prescriptions you are
given
- ask about the best time to call Dr. Salazar with questions about
your vision or eye health
- ask when you need to see us again
